Additional Labs Needed If You Experience Chronic, Recurring SIBO
A SIBO test tells you that bacteria are overgrowing in your small intestine. That’s it. It doesn’t tell you why those bacteria are allowed to overgrow there, or what broke down in the gut's self-cleaning mechanisms that are supposed to prevent that overgrowth in the first place. So treatment gets aimed at killing the bacteria by default, because nothing else was ever investigated.
One of the biggest reasons SIBO keeps coming back is that the investigation stops at SIBO itself. A breath test confirms the overgrowth, and treatment immediately shifts to killing the bacteria, or symptoms alone are used to assume SIBO is present and treatment begins without testing at all. But when SIBO becomes chronic and recurring, treating the overgrowth alone is not enough. The recurrence is a sign that the deeper root cause driving the SIBO has not been identified or addressed, and without the additional testing needed to uncover it, the same cycle of treatment and relapse continues.
Recurring SIBO calls for a closer look
There are several deeper root causes that can create the conditions for SIBO to develop and continue returning, including impaired gut motility, nervous system dysfunction, low thyroid function, maldigestion, poor bile flow, chronic stress, and environmental exposures like mold. And often, there isn’t just one driver. Several systems can be breaking down simultaneously, which is why recurrent SIBO requires looking beyond the overgrowth itself. Stool tests reveal clues about gut function
Here are the additional tests we find most helpful when a client is struggling with chronic, recurring SIBO and what each one can help us understand:
A Comprehensive Stool Test. This is the bare minimum for giving us greater context around why SIBO is present because it allows us to look at the gut environment as a whole and assess how the digestive system is actually functioning. We can assess digestive capacity, pancreatic enzyme output, gut immune function, inflammation, beneficial bacteria, overgrowth patterns, and other markers that help us understand where the digestive environment has broken down. If digestive secretions are impaired, microbial balance has shifted, or immune defenses are depleted, those are important pieces of the picture that can help explain why the gut lacks resilience to maintain its proper self-cleaning mechanisms to prevent SIBO formation and relapse.
Bloodwork can uncover other contributing factors
A Comprehensive Blood Panel. Most blood panels from your PCP are incomplete and don’t give us anywhere near the information we actually want when assessing the systems that influence gut function and motility. At a bare minimum, we want insight into metabolic health, including fasting insulin, to assess whether blood sugar dysregulation and its downstream inflammatory effects may be contributing. We want a full thyroid panel, including Free T3 and thyroid antibodies, because low thyroid activity can directly slow gut motility and increase the likelihood of SIBO formation. We want a full iron panel, not just ferritin, because iron status is closely connected to thyroid function, energy production, and the metabolic processes the gut depends on. And in an ideal world I also want inflammatory, liver, methylation, and nutrient markers such as hs-CRP, homocysteine, GGT, vitamin D, and vitamin B12 to help assess detoxification and oxidative stress that can impact gut health. We can learn a tremendous amount from bloodwork, but only when we are actually running complete enough panels to see the bigger picture.

Testing may reveal additional factors behind recurring symptoms
Organic Acid Testing and Mold/Mycotoxin Testing. Testing is always based on the client’s unique symptoms, presentation, and health history. When there are signs that fungal burden, environmental toxin exposure like mold, or impaired detoxification may be part of the picture, we test deeper because these can be upstream stressors that continue to disrupt the systems responsible for keeping the gut moving and resilient. In our practice, we may use organic acid and toxin testing to assess patterns related to fungal overgrowth, toxin exposure, cellular metabolism, and detoxification. These findings can give us another layer of understanding when someone has already completed multiple rounds of SIBO treatment only to have the same symptoms return.
The value of testing is not just in running more labs. It is in understanding how the findings connect and using them to determine what needs to be addressed, in what order, and whether each phase of treatment is actually accomplishing what we intended it to accomplish. Testing is step one. Interpretation, connecting patterns across tests, and progressing treatment based on how the body responds is the more important and nuanced part that follows.
